I must have these four forms in hand before we can begin working together. While I need every page completed of the Intake Form, I only need the signed final pages of both the Notice of Privacy Practices, the Psychotherapist-Client Agreement Form, and the Informed Consent. It is a good idea to print the last 3 forms and keep a personal copy.
